I have this problem also after a a couple years of use it started in the last 6 months. I have MJ under its own user account in windows 7

It sometimes happens to me when I first dial out or answer. Usually the only remedy is to restart the MJ software and then it works fine - temporarily.

I have the same problem but only occasionally. When MJ starts up it works fine but at some unknown point it happens randomly (sometimes in the middle of a call).

I am using MJ under its own Win7 profile. The only way I can get the audio back is to switch to that profile and restart MJ.
