I'm living in Canada and have family in New Jersey which is what I use my MJ for mostly. I have a local NJ area code, so they can call me for free and vice versa. Love it and it works great.
Anyway, I'd like to forward my incoming MJ calls from New Jersey to my Canadian cell phone.
Thing is, how does that work exactly? Is it still considered a local call for my family in NJ when it is forwarded to my Canadian cell phone or will long distance rates apply once it connects to my cell?
Probably a really simple answer, but I'd just like confirmation.

MJ calls to Canada are also free of long distance charges. So, no, neither your family nor you have to pay long distance charges. Your family calls the NJ number associated with your mj = no long distance. MJ forwards to Canadian number= no long distance charges.

Which number shows up on my cell phone? My MJ number that is forwarding the call or the number of my family in NJ? Still not exactly sure how call forwarding works. I also understand that my MJ doesn't have to be on to have calls forwarded. Is that correct?
And I'd assume that on my cell phone, I'd only pay for regular airtime to receive the call since there's no extra charge for receiving long distance calls with my provider.

I believe you are correct in your assumptions. The number on you cell caller ID will be your MJ number. To call forward go to mymagicjack by clicking the 'my' button on the softphone and set up forwarding. Your MJ does not need to be on for the forwarding to function. Good luck.

So, I ended up trying this with my cell phone and here are the results:
Family in New Jersey called my local New Jersey MJ number with their landline and the call was forwarded to my Canadian cell phone number successfully.
BUT...
The number on my call display was not my MJ number forwarding the call, but rather it was the number of their home landline. I really don't think this is anything to worry about, but I was hoping it would be my MJ number showing up on the call display.
When I pick up, I hope that no long distance charges are incurred by my family.
