Maybe 2 lines, but I could make dew with just one.
I would like for it to be open, for support of DD-WRT
Does anything like that exist? I am currently using a Netgear WNR834B v2 with DD-WRT and a PAP2-NA.
Any ATA-Router combo devices?

I don't think you will find that whole feature list in one box. This is because DD-WRT does not have any capability of driving whatever ATA would be built into such a router/ATA combination box.
And, please don't make dew with one line. If the dew seeps into the router you will fry the electronics inside.

It does have the ability, as it is open source 
There is even VOIP firmware available, though I have never used it.
