-Local inbound/outbound calls to same 480 area code work fine
-dial plan in ATA:
(911S0|<:1>[2-9]xxxxxxxxxS0|1[2-9]xxxxxxxxxS0|011[2-9]x.)
When a long distance number is dialed, both using 10 and 11 digit formats, a fast busy signal is heard.
This same config worked for months, now no long distance???
could someone give some insight??

Fast busy signals indicate dial plan problem. Try the one below and see if that helps at all. This should work with 7, 10, and 11 digit dialing, and should work fine for international calling.
(*xx|[3469]11|0|00|<:1480>[2-9]xxxxxx|1xxx[2-9]xxxxxxS0|<:1>[2-9]xx[2-9]xxxxxxS0|xxxxxxxxxxxx.)
For a general explanation of how a dial plan works, here is an example:
*xx = allow star codes (*67, *81, etc.)
[3469]11 = allow 311, 411, 611, 911
<:1480>[2-9]xxxxxx = allow 7-digit dialing in the 480 area code.
1xxx[2-9]xxxxxxS0 = normal 1 + 10 digit North America numbers. S0 means no delay.
<:1>[2-9]xx[2-9]xxxxxxS0 = allow 10 digit dialing for North America numbers. S0 = no delay.
xxxxxxxxxxxx. = any long number... international numbers basically.
