Configure AC 211N SR SunRocket ATA

I have unlocked my old AC 211N SR SunRocket ATA (SIP version 4.60 US), but am having trouble entering my SIP info. My menu options and screens seem a bit different than most referenced in the instructions here and other tips I can find via Google. For instance, I don't see an option to disable provisioning.

I think I am "so close and yet so far". Any help would be appreciated by this newbie.

Perhaps it shows up on another page. It appears that there are several.

All I did was enter AC211 and look what popped up!!
