Need Sipura-compatible dial plan

Need someone to create a Sipura-compatible dial plan with following characteristics:

If I enter just 7 digits I want it to prepend area code 773 to that number

Allows me to dial 10 digit numbers with or without the "1" country code

Does not allow calls outside the USA

Blocks all pay-per-minute commercial area codes such as 976, etc

Allows dialing 311 to connect to 3127445000
Allows dialing 411 to connect to 8004664411
Allows dialing 911 to connect to 3127444000
