Can someone help me figure out where to enter the information on my DVG-2001S VoIP Phone Adapter?
I have my username and password for MJ.

The info you're looking for should be located under the advanced/voip tab (select advanced on the top and voip on the left menu). You'll need your SIP information rather than the username/password combo that you signed up with. The following thread contains information on how to get your SIP information:

I got this working on my DVG-2001S. I have been using it for a couple of days and have made/received maybe 10-15 calls without any problems and with better sound than I used to have with my Bellsouth line. I am also using a perl script I found in another thread running on my Fedora server to spoof my caller ID with my grandcentral.com phone number since MagicJack does not have my area code available yet (see this thread for info on how to do this: Thread describing caller id spoofing)
Using Fiddler I got the following info for my MagicJack account (the xxxx's are there to avoid showing my actual info):
